Follow up and tracking of MB and PB leprosy cases

      Follow up and tracking of MB and PB leprosy cases and

      Implement logic for Completing treatment of PB (Paucibacillary) and MB (Multibacillary) leprosy patients. Incentive eligibility should be determined based on leprosy type and treatment start date, follow-up duration and treatment completion date.

      Requirements:

      : follow up for Leprosy conformed cases

      In the follow up screen following changes are required:

      • Allow date back up to 3 months from current month
      • Not greater than Today's Date 
      2 Expected Treatment Completion Date Label Read only Auto calculate this date based on below condition form ‘Treatment Start Date’:
      If type is PB case Treatment duration is 6 months
      If type is MB case Treatment duration is 12 months
      3 Follow up Date Date picker
             		   §  Greater than or equal to Treatment Start Date
      §  Minimum date is Treatment Start Date
      §  Accept date greater than Last Follow up Date
      §  Allow 1 Follow up visit in a month (i.e. monthly 1 Follow up)
      §  Not greater than Today's Date
      And next Follow up date should be greater than previous Follow up date
        MDT Blister Pack Received?      
        Treatment Status Dropdown    
        Actual Treatment Completion Date Date picker   Enable this filed based on below conditions:
      1. if 'Treatment Status' is “Treatment Completed” and
      2. If ‘leprosy Type’ is “PB” then enable after 5 Monthly follow up visits
      3. If ‘leprosy Type’ is “MB”, then enable after 11 Monthly follow up visits

      Date should be greater than or equal to Last Follow up Date|

       

      • After conformation Treatment tracking of Visit cycle starts with multiple follow up visits, i.e. monthly 1 Follow up visit is required.
      • Provide edit option to add follow up visits
      • If type is PB case Treatment duration is 6 months and eligible for incentive
      • If type is MB case Treatment duration is 12 months and eligible for incentive
      • Tracking of Visit-1 cycle closes after ‘Actual Treatment Completion Date’
      • Once Treatment status is cured, Visit-1 cycle closes and record is available for further screening
      • In the conformed section, once Treatment is started, show below details on the Beneficiary card:
      1. Treatment is start date
      2. Expected Treatment Completion Date
      3. Last/Previous Follow-up Date
      4. Next Follow-up Date
