Following Modifications required in cataract surgery form:

1. Rename Eye surgery to Cataract Surgery
2. Title name: Cataract Surgery with Eye icon
3. Provide this option form Age 40 years and above for both Males & females (currently form 30 years)
4. Allow Date back up to 1 month in Visit and surgery dates
   Symptoms: make multi select
5. Referred to: Rename Public facility to Govt Public facility
6. Referred cases, should be added to HWC Referral list (Reason for visit & Type is Cataract Surgery) and send current date as Referral date
7.  Referral date is current date
8. Show history of referral / surgery based on visit date and maintain visit id
9. And maintain the history of surgery for each eye and both eyes based on selection
10. Maintain a history table at bottom of the screen with edit option for every visit (maintain one visit for one eye)
11. If any Eye left or Right or both surgery is done or referred – flag it as done and do not allow to add repeatedly
12. One change for one eye, if both eyes are completed, don’t provide add option only edit option
13. Follow up status should be non-mandatory and (add Suspected & Referred)
14. Provide the option for File upload (allow up to 2 files) - (Only Applicable for Utprerona build variant)
