Update the existing ANC form to track PMSMA visits for high-risk pregnant women. Assistance should cover up to 3 visits to a government hospital under PMSMA. Include additional fields to capture the place of ANC visit and PMSMA-specific visits for proper reporting and monitoring.

Requirements:

• Update the existing ANC form to include the following fields:
• Place of ANC Visit (dropdown values:
  • Sub-Centre
  • VHND/ VHSND
  • PHC

  • PMSMA Visit

• • CHC
  • District Hospital
  • Medical College Hospital

1. If 'Place of ANC Visit' is "PMSMA Visit" then, show PW in PMSMA visit line listing
2. Remove existing Add and View PMSMA/ ePMSMA form in both sections ANC and ePMSMA
3. and in ePMSMA section replace the Add and View PMSMA visit form with exiting ANC visit form whare 'Place of ANC Visit' should be only "PMSMA Visit" and by default selected 
4.  in ePMSMA section in 'View PMSMA visit' show only the visit and count of  'Place of ANC Visit' is "PMSMA Visit"

• Track high-risk pregnant women separately
• Data captured should reflect in reports for PMSMA program monitoring
• Ensure workflow accommodates up to 3 PMSMA visits per patient
