Implement 6-monthly follow-up for BP, Diabetes, and Cancer patients

      Enable tracking and follow-up of patients diagnosed with BP, Diabetes, and Cancer on a 6-monthly schedule. The system should allow logging follow-up visits, capturing key health information, and supporting reporting and monitoring.

      Requirements:

       In the NCD referral list on every card show a button with title “Follow up”

      an on click show below field and submit for next follow up:

      1. Diagnosis: Hypertension (BP), Diabetes (DM), Cancer (Multi-select Dropdown)
      2. Treatment Start date: (allow back date up to 6 months and Not greater than Today's Date)
      3. Follow up Date: (Date greater than or equal to 'Treatment Start date' and Not greater than Today's Date)
      4. File upload: 
      5. Maintain the Visit cycle with sub set of follow up visits (every Visit cycle has follow up visits)
      6. Maintain a table with visits and follow up visits in the bottom of the screen
      7. Follow up interval: Monthly one Follow up visit up to 6 months
