Set Default Values for Maternal Death and Abortion Fields to 'No'

      As an ASHA worker, I want the fields for Maternal Death and Abortion to default to "No" when creating or updating records, so that these fields are automatically set to the most common response, minimizing user input errors.

      Acceptance Criteria:

      • Default Value for Maternal Death Field:
        • The "Maternal Death" field should default to No when a new record is created or when the field is initially displayed.
        • If the user has not selected an option, "No" should be pre-selected.
        • If the user selects a different option (Yes/No) during editing, it should update accordingly.
      • Default Value for Abortion Field:
        • The "Abortion" field should default to No when a new record is created or when the field is initially displayed.
        • If the user has not selected an option, "No" should be pre-selected.
        • If the user selects a different option (Yes/No) during editing, it should update accordingly.
