Display Previous Visit History Table and High Risk Conditions in ANC Module Form

      As a healthcare provider, I need a Previous Visit History Table displayed below the Name and Gender bar in the ANC visit form. This table will show essential information from the previous ANC visit, including weight, blood pressure, hemoglobin, fundal height, and blood sugar. Additionally, a field should be added above the "No of IFA Tablets" to display the previous visit values. If High Risk Conditions were marked Yes in the previous visit, this field should display Yes by default and be editable to change to No if necessary. I should also be able to view and update the High Risk Condition field based on the previous visit's value.

      • The Previous Visit History Table should be displayed below the Name and Gender bar in the ANC visit form.
      • The table should display the following columns, populated with data from the most recent ANC visit:
        • Weight (kg)
        • Blood Pressure (BP) (mmHg)
        • Hemoglobin (Hb) (g/dL)
        • Fundal Height (cm)
        • Blood Sugar (mg/dL or mmol/L)

      Acceptance Criteria:

      • If any of the fields (weight, BP, Hb, fundal height, blood sugar) from the previous visit are missing, display Blank in the respective field.
      • A field should be added above the "No of IFA Tablets" field to show the previous visit values (weight, BP, Hb, fundal height, blood sugar).
      • If the "Any High Risk Conditions" field was marked "Yes" in the previous visit, it should display "Yes" in the current visit by default. This field should be editable to change to "No" if applicable.
      • The "High Risk Condition" field should display the value from the previous visit and allow the healthcare provider to update it if required.
      • All fields displaying previous visit data (weight, BP, Hb, fundal height, blood sugar) should be read-only, unless explicitly editable (such as the "High Risk Conditions" and "High Risk Condition" fields).
      • The Previous Visit History Table should automatically fetch and display the most recent data for the patient from the system.
      • The form and table should be responsive and display correctly across mobile and tablet screens.
      • The Previous Visit History Table and related fields should be positioned clearly at the top of the form for easy access, without cluttering the page.
