Requirements

1. Edit TB Screening Form
   • Update the existing TB Screening form as per the referenced prototype.
   • Ensure the fields, sequence, labels and workflow are aligned with the prototype.

Acceptance Criteria

• TB Screening form is updated as per the referenced prototype.
• No changes are made to subsequent TB modules as part of this story.
