As a Volunteer
I want to initiate TPT for contacts who are eligible and advised
So that they begin preventive treatment with the correct regimen and expected completion date

Acceptance Criteria

• [ ] "Are you advised to take TPT" — Yes/No. No → go straight to Submit
• [ ] TPT initiated — Yes/No; mandatory when advised; Yes enables remaining fields
• [ ] TPT start date — mandatory when initiated; no future date; at least screening date
• [ ] Treatment duration — radio: 1 / 3 / 6 months
• [ ] Expected completion date — auto-calc (Start + duration), not editable
