Consent should be added during Household Registration (same as Family Member Registration)

      Currently, while registering a Household, the Consent Form is not being shown. However, during Family Member Registration, a consent popup is displayed and requires user approval.

      Expected Result

      • Consent Form popup should be displayed during Household Registration
      • User must Agree/Disagree before proceeding
      • Behavior should be consistent with Family Member Registration
