As a Volunteer
I want to record treatment completion and outcome for a confirmed TB case
So that programme-wide outcomes (cure / failure / LFU / death) are complete and death feeds the beneficiary record

Acceptance Criteria

• [ ] "Did the patient complete the full course?" — Yes/No, enabled only after regimen-specific month thresholds are met (DS-TB/BPaL/INH Mono after 5 visits; Shorter after 9; Longer after 18)
• [ ] Actual Treatment Completion Date — enabled when Yes; must be after Last Follow-up Date
• [ ] TB Treatment Outcomes — dropdown: Cured / Failed / Lost to Follow-up / Death
• [ ] If outcome=Death: auto-set Beneficiary Status=Death and capture Date, Place, Reason (read-only "Tuberculosis")
• [ ] Date of Death — mandatory when outcome=Death; no future date; at least Treatment Start Date or after Last Follow-up Date
• [ ] Place of Death — dropdown enabled only when outcome=Death (Home / Subcenter / PHC / CHC / District Hospital / Medical College Hospital / Private Hospital / Other Place)
• [ ] Reason for non-completion — textbox, enabled when completion=No
