Multiple Validation and Data Issues in Follow-Up Module

      Multiple issues observed in the Follow-Up module related to validation, field behavior, and default values:

      1. Follow-Up Date Validation Missing:
        • No validation present for follow-up date.
        • System allows invalid or incorrect date selection.
      2. Actual Treatment Completion Date Error:
        • Error is displayed on second click of submit button even when data is correct.
      3. Validation Missing for Date of Death Field:
        • No validation applied on Date of Death field.
        • Incorrect or empty values are allowed.
      4. Reason for Death Not Prefilled:
        • "Reason for Death" field is not auto-filled as "Tuberculosis" for TB beneficiaries as expected.
      5. Submit Button Behavior Issue:
        • Submit button remains enabled even after completing follow-up.
        • It should be disabled once follow-up is submitted to prevent duplicate entries.

      Steps to Reproduce:

      1. Navigate to Follow-Up module.
      2. Fill form with different combinations:
        • Leave date fields invalid/empty.
        • Submit form multiple times.
        • Check death-related fields.
      3. Observe validation behavior and button state.

      Expected Result:

      • Proper validation should be applied on:
        • Follow-Up Date
        • Actual Treatment Completion Date
        • Date of Death
      • "Reason for Death" should be auto-filled as "Tuberculosis" where applicable.
      • Submit button should be disabled after successful submission.

      Actual Result:

      • Missing validations on key fields.
      • Incorrect error behavior on second submit.
      • Reason for Death not prefilled.
      • Submit button remains active after submission.
