As a Volunteer
I want to record temperature, pulse, blood pressure and random blood sugar
So that out-of-range values trigger an HWC referral alert

Acceptance Criteria

• [ ] Temperature (°F) — free textbox AND preset options: 97.5 · 98.5 · 99.5 · ≥ 100
• [ ] Alert "Refer to Health and Wellness Centre" if Temperature ≥ 100 °F
• [ ] Pulse Rate (BPM) — free textbox AND preset options: under 60 · 60–70 · 70–80 · over 90
• [ ] Alert "Refer to HWC" if Pulse under 60 or over 90 BPM
• [ ] Systolic BP (mmHg) — free input; alert HWC referral if ≥ 140 or under 90
• [ ] Diastolic BP (mmHg) — free input; alert HWC referral if ≥ 90 or under 60
• [ ] Random Blood Sugar (mg/dL) — free input; alert HWC referral if ≥ 100
• [ ] All fields optional (screen can be skipped)
