As a Community Health Officer (CHO), I want to record elderly health assessment details for patients aged 60 years and above, so that functional decline and memory-related issues can be identified and referred appropriately.
Acceptance Criteria
- Section is visible only if patient age ≥ 60 years
- “General geriatric complaints” is mandatory
- “Functional decline” and “Memory loss” are mandatory
- Functional Assessment section auto-enables if functional decline = Yes
- Dementia Screening Checklist auto-enables if memory loss = Yes
- System shows referral alert if:
- Functional decline = Yes OR
- Memory loss = Yes
- Optional checkboxes do not block form submission
