      As a Community Health Officer (CHO), I want to record elderly health assessment details for patients aged 60 years and above, so that functional decline and memory-related issues can be identified and referred appropriately.

      Acceptance Criteria

      • Section is visible only if patient age ≥ 60 years
      • “General geriatric complaints” is mandatory
      • “Functional decline” and “Memory loss” are mandatory
      • Functional Assessment section auto-enables if functional decline = Yes
      • Dementia Screening Checklist auto-enables if memory loss = Yes
      • System shows referral alert if:
        • Functional decline = Yes OR
        • Memory loss = Yes
      • Optional checkboxes do not block form submission
