Simplify the referral process and make comprehensive referral section

This new referral section should contain all referrals from entire modules with option to filter by ‘Referral Type’.

• Rename “NCD Referred list” to “HWC Referrals”
• And move this “HWC Referrals” section to Home screen (in place of Communicable Diseases)
• Move ‘Communicable Diseases’ section to ‘Disease control’ section
• Rename ‘Communicable Diseases’ to “Tuberculosis (TB)”
• Place “Tuberculosis (TB)” and “Leprosy” sections in one row

Note:

This “HWC Referrals” section, should contain all referral types like NCD cases, TB, Leprosy, Malaria, ANC (HRPW), Cataract, Cancer, Geriatric, COPD, Depression, SAM, PNC, etc

Changes required in the NCD >> CBAC Form:

Currently in the CBAC Form to refer navigates to a Referral screen, instead with in the CBAC Form, under each section (part-A, B, C) show referral details (Referral Facility, Reason for Referral & Referred date), based on the score or suspect then make referral as mandatory and validate to select “Referral Facility”.

In the referral details, these are following changes:

• Rename “Higher healthcare center” to “Referral Facility”
• Rename “Referral Reason” to “Reason for Referral”
• Rename “Refer date” to “Referral Date”
• Referral Facility drop down should have these item values: Subcenter, PHC, CHC, District Hospital, Medical College Hospital
• All these referral beneficiaries should show in the “HWC Referrals” section, with respective “follow up” (for NCD cases), “Suspected” (TB, Leprosy, Malaria, etc) button
