Add "None of these" functionality in Part B to allow changing individual fields, add demographic details at the top, and apply alternating colors for Parts A, B, C, D for improved readability.

Acceptance Criteria:

• Add "None of these" Field: In the CBAC ** (Community Based Assessment Checklist), under Part B, when the field "None of these" is selected, all the fields are automatically marked as No. However, if the user needs to change any of these fields to Yes, the system should allow this adjustment.
• Add Demographic Details on Top: Ensure that the demographic information (such as Name, Age, Gender, Ben id) is displayed at the top of the form or screen for easy reference before moving into the main sections.

• Alternate Colors for Parts A, B, C, D: Apply alternating background colors to distinguish each part (A, B, C, D) for better readability and visual separation.
