Identify and remove some fields from the existing ANC form to simplify data collection.

1. Data Fields to be made Non-mandatory:

1. BP of PW
2. HB

Note: This above 2 fields are Mandatory only for Xushrukha App and Non-mandatory for Utprerona and Mitanin App.

2. Data Fields to be hide in UI form:

1. Pulse Rate
2. Fundal Height
3. Urine Albumin
4. Random Blood Sugar Test

3. "Add ANC" should be shown disabled when till due date (currently it is hidden) 

4. Note: MCP Card file upload is not required for Mitanin App.

Acceptance Criteria:

• List of unnecessary fields is finalized and approved

• Approved fields are removed from the ANC form

• Existing data and validations are not impacted

• Clinical inputs need to remove from the form
