Allow users to choose whether they want to continue the ANC visit using the new checklist or the existing ANC form.

Acceptance Criteria:

• User can select either:
  • New ANC Checklist, or
  • Existing ANC Form
• Selected option remains consistent during the visit
• Existing form works without disruption
