1. In the NCD Referral in the 'Reason for refer' should have following values based on the section:

1. Part A: Risk Assessment: reason for refer should be "Suspected NCD Case"; and Type: NCD
2. Part-B1: reason for refer is "Suspected TB case" for TB related questions and Type: TB
3. And Part-B1: reason for refer is "Suspected Leprosy case" for leprosy related questions and Type: Leprosy;
4. Part B3: reason for refer is "Geriatric screening" and Type: Geriatric
5. Part-C: reason for refer is "Suspected COPD case" and Type: NCD
6. Part-D: reason for refer is “further assessment for depression” Type: Geriatric

Note: Incentive related to "Follow-up of BP, diabetes, and cancer patients every 6 months"

Additional Changes related to Alerts & dialog box :
Actually there is an existing alert box and new referral dialog box, instead of these two better to show one dialog box in every section section.
then for example the dialog box message should be:

"Suspected NCD case! Do you want to refer to HWC facility?" with "No" or "Yes" options.

"Suspected TB case! Do you want to refer to HWC facility?" with "No" or "Yes" options.

"Suspected Leprosy case! Do you want to refer to HWC facility?" with "No" or "Yes" options.
