      As an ASHA, I want to receive an incentive of ₹150 per SAM case, so that I am motivated to identify, refer, and follow up with children suffering from Severe Acute Malnutrition (SAM), ensuring they receive proper care and do not relapse.

      Description: 

      SAM (Severe Acute Malnutrition)

      ASHA is responsible for identifying SAM cases in the community for the infants and children who are 6 to 59 months of age and have a mid-upper arm circumference (MUAC) <115 mm (<11.5 cm), or a weight-for-height/length <-3 Z-scores of the WHO Child Growth Standards median, or have bilateral pitting oedema, should be referred for full assessment at a treatment center (NRC) for the management of severe acute malnutrition. 

      Acceptance Criteria

      S.No. Criteria Details
      1 SAM Identification Child is marked as SAM if MUAC ≤11.5 cm or Weight-for-Height Status = "SAM".
      2 Referral to NRC ASHA must select "Yes" for referral if SAM is identified.
      3 NRC Admission Date Must be entered and valid (after child's DOB and before or on today's date).
      4 Follow-up Visit Date Must be entered and valid (after DOB and before or on today's date).
      5 Follow-up Outcome Outcome must be recorded as Improved, No Change, or Relapse.
      6 System Calculated Incentive If all 3 conditions are fulfilled (Referral + Admission + Follow-up), system sets Incentive Status = "Eligible". Otherwise, "Not Eligible".
      7 Claim Form Mapping The incentive appears under Child Health > S. No. 4 in the ASHA Master Claim Form.
      8 Amount ₹150 per eligible SAM case.
