1. Incentive Logic

• ASHA will receive ₹150 per case as an incentive after the ‘Follow-up Visit Date’ is submitted.

• Applicable only if the full Referral → Admission → Follow-up cycle is completed.

• Incentive should be recorded under Child Health → S. No. 4 → “ASHA incentive for referral of SAM cases to NRC and for follow-up of discharged SAM children from NRC”.

• Once eligibility is met, the record should be available in the Incentive Master Claim form.

2. Dynamic Form API

Since the SAM form is dynamic, backend API support is required for:

• Form metadata (fields, field types, validations, conditions, labels, dropdown values, etc.)

• Form submission (create/update visit cycle data)

• History maintenance (each visit entry, status update, and follow-up record)
