As an ASHA, I want to be able to fill and submit a Master Incentive Claim Form covering all 84 approved activities, so that I can claim my earned incentives accurately and transparently for the services I provide.

Acceptance Criteria:

Form Structure:

• The form should support entry for all 84 approved ASHA incentive activities.
• Each activity should be pre-listed with:
  • Activity name
  • Fixed rate (₹)
  • FMR code
• Data Entry Fields per Activity:
  • Number of services rendered (per activity)
  • Auto-calculated total amount (Rate × Count)
  • Document attachment field (if applicable)
  • Optional remarks field
• Summary Section:
  • Auto-sum of all claimed amounts
  • Monthly claim period (start and end dates)
  • Village and Block level identifiers
• ASHA & Verifier Info:
  • ASHA Name, Bank Details, Phone Number
  • ANM/Supervisor Verification fields
  • Digital Signatures or checkbox for declaration
