Need a user interface for the ASHA Claim Form that lists all tasks and automatically fills in the incentive amount for tasks where the ASHA worker is eligible, leaving other tasks blank.

Acceptance Criteria:

1.Display the ASHA Claim Form

• Create a user interface that lists all the tasks
• Fill in the Amount in respective tasks for which an ASHA worker is eligible to receive incentives.

2.Eligibility Check

• Implement logic to check the eligibility criteria for each incentive.
• Display only the eligible incentives based on the ASHA worker's completed activities and other predefined conditions.

3.Calculate Incentives

• Allow users to select a date range to view the incentives.
• Automatically calculate the total incentives based on the selected date range and eligible activities.
• Display the calculated total on the form.

4.Download the Form

• Add a feature that allows the ASHA worker to download the claim form
