As an ASHA user, I want to report child death cases through the CDR (Child Death Reporting) form, so that the FLW App can capture the child's status, verify age eligibility (under 15 years), and calculate incentives accordingly.
Condition: Show list of beneficiaries in the CDR line listing section where Infant Death or Child Death is noticed, between Age group 0 to less then 15 years..
i.e. List of beneficiaries, in the CDR line listing is form death is reported from HBNC or HBYC or Beneficiary section.
Death should be flagged in the beneficiary line list.
Enable below fields in FLW App:

Other fields in the CDR form should be auto filled as per the BRD.
Acceptance Criteria:
If any "Child death" is reported, update 'Beneficiary Status' = "Death" in the Beneficiary record, with “Date of Death ”, "Place of Death" and “Reason for Death” in Beneficiary table and sync to AMRIT
New Field - Status:
- A new field labeled "Beneficiary Status" must be added to the beneficiary form.
- Select options: Alive and Death.
- Default selection will be Alive
- The field is mandatory.
- Other fields in the CDR form should be auto filled as per the BRD.
Age Validation:
- The system should allow child death reporting only if the age of the child is less than 15 years.
File Upload Option:
- A file upload field should be available in the CDR form.
- Allowed file types: PDF, JPG, PNG (configurable).
- File size limit: [Max size, e.g., 5MB].
- File is mandatory if status is selected as "Death"
Incentive Calculation:
- Once a death is reported (Status = Death) and age < 15, the system should automatically flag the case for ASHA incentive eligibility with 100 Rs per case.
- Details should be logged for further approval/review by the concerned authority.
Audit Trail:
- All submissions should log the user ID, timestamp, and status selected for traceability.
