In the General Examination section under Vitals, while entering Blood Pressure (BP), the system repeatedly prompts for referral even after entering a single value. Additionally, there are no validations for BP input. After submission, the OK button does not respond on the first click and requires clicking twice to proceed.

Steps to Reproduce:

1. Navigate to Beneficiary → General Examination section.
2. Go to Vitals and enter BP value.
3. Observe the referral prompt behavior.
4. Submit the details.
5. Click on the OK button.

Expected Result:

• BP entry should trigger referral prompt only when applicable (based on valid conditions).
• Proper validations should be present for BP input.
• OK button should respond on a single click.

Actual Result:

• Referral prompt appears repeatedly even after entering a single BP value.
• No validations available for BP input.
• OK button requires double click to proceed.
