Add "Clinical Assessment and Diagnosis" referral queue to Referrals module

      As a Nurse / Lab Technician, I want a "Clinical Assessment & Diagnosis" card in the Referrals section listing beneficiaries whose digital chest X-ray was positive but whose NAAT was negative, so that the abnormality detected on the X-ray can be further investigated at the PHI rather than the case being dropped.

       Requirement
      Add a fifth card to the Referrals module, alongside the existing Digital Chest X-ray, NAAT, Liquid Culture and Health and Wellness Centre cards.

      Routing rule: beneficiaries with a Positive digital chest X-ray but a Negative NAAT result (MTB not detected) are shown in Clinical Assessment and Diagnosis in the referral section.

      Acceptance Criteria
      "Clinical Assessment and Diagnosis" card renders on the Referrals screen with a count, alongside the existing four cards
       Tapping the card opens the referral list screen using the common list elements shared by the other referral cards
      List auto-populates with beneficiaries where Digital Chest X-Ray Test Result is TB Presumptive / Positive AND NAAT MTB Test Result is MTB Not Detected
       Each row deep-links to the beneficiary record
      Beneficiary leaves this list if a later diagnostic result changes their classification
      Card is visible per role-based access already defined for the Referral section (Nurse, Counsellor, Lab Technician)
