As a Volunteer
I want to close a TPT case with its completion outcome (date of completion, or date and cause of death)
So that each TPT case has a definitive end-state for reporting

Acceptance Criteria

• [ ] Date of completion — mandatory when status=Completed; no future date
• [ ] Date of death — mandatory when status=Died during TPT; no future date
• [ ] Cause of death — free text, mandatory when status=Died during TPT
• [ ] Geolocation captured on Submit
• [ ] Submit closes the TPT case; no further edits unless re-opened
