As a Volunteer
I want to record follow-up visits during TPT
So that adherence and outcomes are tracked, with the right routing when active TB develops or a contact dies

Acceptance Criteria

• [ ] Follow-up visit number — numeric, auto-increment 1..6, not editable
• [ ] Follow-up visit date — mandatory; no future date; at least TPT start date
• [ ] TPT completion status — options: Completed / Incomplete / Lost to follow up / Developed active TB during TPT / Died during TPT / Other
• [ ] "Completed" enables Date of completion
• [ ] "Developed active TB during TPT" → create a new active-TB case record (STOP-5) and close this TPT case
• [ ] "Died during TPT" enables Date of death and Cause of death (free text)
• [ ] "Other" enables free-text reason
