In TB screening form-
 
1. Date of visit to be removed from Ui and to be managed from backend.
 
2. "Check if the Person has any of these symptoms" before the questions. "" and "" alerts should be put after "" and "**" and before the list of symptoms.
 
3. Add a new title/ section "History of TB" before the following three questions- History of TB, Are you currently taking Anti-TB drugs, Anyone in Family Currently Suffering from TB.
 
4. Give a search Bar in the Key Population / Risk Factors section and also remove "Not Applicable" as a default. Important for field team to capture this.
 
5. Remove "unknown" as default from HIV status.
 
6. Remove "Unknown" as default from Occupation field. The field team needs to capture these details authentically.
