      User Story

      As a Volunteer
      I want to record monthly follow-up visits (adherence, discomfort) for a confirmed TB case
      So that the programme can track progress and catch drop-offs

      Acceptance Criteria

      • [ ] Follow-up Date picker — mandatory; at least Treatment Start Date; strictly after Last Follow-up Date; no future date; only one follow-up per month
      • [ ] Monthly follow-up counter label — Month-1 to Month-24, auto-incremented from Start Date
      • [ ] Adherence to Medicines — radio: Regular / Irregular
      • [ ] Any discomfort — radio: Yes / No
