      Currently TB screening form filling is mandatory before proceeding further. Now following change is needed.

      1. Enable both TB screening and Anthropometry forms in the bottom sheet.

      2. Rename following titles and lables:
      a. "TB Screening form" to "TB Symptoms Screening"
      b.  "Anthropometry screen" to "Anthropometry "
