There is a mismatch between the Counselling module implementation and the PRD.

• The application is displaying additional/unnecessary questions in the Counselling module.
• As per PRD, only specific questions/fields should be shown in this section.
• Current UI includes extra questions which are not defined in PRD.

Steps to Reproduce:

1. Navigate to Counselling module for a confirmed TB beneficiary.
2. Open the Counselling form.
3. Observe the list of questions displayed.
4. Compare with PRD-defined questionnaire.

Expected Result:

• Only Counselling Consent should be displayed.
• No unnecessary questions should be visible.

Actual Result:

• Extra questions are displayed which are not part of PRD.
• UI does not match PRD specifications.
