Counselling Data Loss After Submission, Validation Error Persist & Screen Stuck

      Multiple critical issues observed in Counselling module after form submission:

      1. Data Loss Issue:
        • After submitting counselling (Sections A–E), when revisiting the form, all data appears blank.
      2. Validation Error Not Refreshing:
        • Even when data is already filled/selected, validation error messages ("This field is required") are still displayed.
        • Due to this, the system behaves as if fields are empty.
      3. Navigation Blocked:
        • "Next" button remains disabled or non-functional.
        • User is unable to proceed to next section.
      4. Fields Not Editable / Screen Stuck:
        • Fields are not editable after reopening.
        • Screen gets stuck with no way to proceed or correct data.

      Steps to Reproduce:

      1. Complete all counselling sections (A–E).
      2. Submit the counselling form.
      3. Reopen/view the counselling record.
      4. Observe:
        • Data visibility
        • Validation messages
        • Button behavior

      Expected Result:

      • Submitted data should be displayed correctly.
      • Validation messages should disappear once fields are filled.
      • "Next" button should work normally.
      • Fields should either be:
        • Editable (if allowed) OR
        • Properly shown in read-only mode with data visible.

      Actual Result:

      • Data appears blank after submission.
      • Validation errors persist even when data is filled.
      • "Next" button not working.
      • Fields are not editable.
      • Screen becomes stuck.
