These issues prevent completion of the follow-up workflow.

Steps to Reproduce:

Issue 1:

1. Navigate to TB Confirmed → Follow-Up module.
2. Select a case where "Death" is applicable.
3. Try to select a value in "Reason for Death" field.

Issue 2:

1. Fill all required fields in Follow-Up form.
2. Click Submit.
3. Observe form behavior.

Expected Result:

• "Reason for Death" field should be selectable and allow user input.
• Follow-Up form should submit successfully when all required fields are filled.
• User should see confirmation or be navigated accordingly.

Actual Result:

• "Reason for Death" field is not selectable.
• Form does not submit despite valid inputs.
• No success message or action observed.
