There are multiple issues observed with notes fields across counselling sections:

1. Data Leakage Issue:
   Notes entered in Section A are incorrectly appearing in Section B notes field.
2. Data Reset Issue:
   When navigating between sections, if a section has no notes entered, previously entered notes disappear and fields show blank.

Steps to Reproduce:

1. Navigate to Counselling → Section A.
2. Enter notes in "Disease Awareness notes".
3. Click Next → Go to Section B.
4. Observe notes field in Section B.
5. Navigate across sections (A → B → C → Back to B/A).
6. Leave notes blank in any section.
7. Return to previously filled section.

Expected Result:

• Each section should maintain its own independent notes.
• Notes from one section should NOT appear in another section.
• Previously entered notes should persist when navigating back and forth.
• Blank sections should not override or reset existing notes.

Actual Result:

• Section B is displaying notes from Section A.
• Notes disappear (reset to blank) when navigating between sections.
• Data is not consistently persisted per section.
