Section Notes Not Persisting – Displaying Blank After Save in Counselling Form

      The notes entered in different sections (e.g., Disease Awareness notes, Do's & Don'ts notes, Schemes notes, Treatment Regimen notes) are not being saved properly.

      After entering values in the notes fields and submitting the form, when the user revisits the same record, all notes fields appear blank instead of displaying the previously entered data.

      Steps to Reproduce:

      1. Navigate to Counselling module.
      2. Enter data in any/all section notes fields.
      3. Click Submit.
      4. Reopen the same beneficiary record.
      5. Observe the notes fields.

      Expected Result:

      • All entered notes should be saved successfully.
      • Notes should be displayed correctly when reopening the record.

      Actual Result:

      • Notes fields are blank after reopening the record.
      • Entered data is not persisted.
