On TB screening form, Date of Visit field is changing according to the current date even after submitting the form.

Steps to reproduce:-

1. Complete TB screening of a beneficiary.
2. Observe the Date of Visit field.
3. Now Change the device time.
4. Now go to TB screening and observe the Date of Visit field.

Expected Result:-

Data should be save according to form submitting date. It should not change according to current date.

Actual result:-

Data of Visit field is changing according to current date when form is already submitted.
