As a health worker/admin, I want the beneficiary registration page to display complete and correctly ordered fields, with location details, so that I can register beneficiaries accurately and efficiently
These details should be customizable and can be manage from admin.
- First Name
- Last Name
- Age
- Gender
- Phone Number
- Marital Information (optional – to be implemented later)
- Spouse Name
- Father's/Husband's Name
- Location Information
Acceptance Criteria
All required fields are displayed on the beneficiary registration page in the following order:
- First Name
- Last Name
- Age
- Gender
- Phone Number
- Spouse Name
- Father's/Husband's Name
- Location Information
Fields are correctly labeled and match the specified order above.
Marital Information field is not yet displayed, as it is to be implemented later.
Location Information must include the following subfields (customizable if needed):
- State/Province
- District
- Sub-district/Block
- Village/Town
