Fields & Logic:

• Gum Diseases Present (Yes/No, mandatory)
  • If Yes → enable Symptoms of Gum Diseases
• Symptoms of Gum Diseases (Dropdown, multi-select, mandatory if enabled)
  • Options: Foul smell, Bleeding gums, Deposits and discoloration, Loose teeth, Widening gap, Swollen gums
  • Referral alert to Dentist if any symptoms selected

Acceptance Criteria:

• Gum Diseases field is mandatory.

• Symptoms field enabled only if Gum Diseases = Yes.

• Symptoms field mandatory if enabled.

• Multiple selections allowed.

• Referral alert triggers for all selected symptoms.
