Fields & Logic:

• Tooth Decay Present (Yes/No, mandatory)

• • If Yes → enable Symptoms of Tooth Decay
• Symptoms of Tooth Decay (Dropdown, multi-select, mandatory if enabled)

• • Options: Black spot, Discoloration of tooth, Cavity, Hole in the tooth, Sensitivity to hot/cold/sweet/sour, Food lodgment, Pain, Swelling, Pus discharge
  • If any symptoms selected → show referral alert to Dentist

Acceptance Criteria:

• Tooth Decay field is mandatory.
• Symptoms field appears only if Tooth Decay = Yes.
• Symptoms field is mandatory if enabled.
• Multiple symptoms can be selected.
• Referral alert triggers when symptoms are recorded.
