As a CHO, I want the Case Identification module to appear only when the patient is symptomatic or has reduced visual acuity, so that I only assess eligible patients.

Acceptance Criteria:

• Module enabled if “Reason for Visit” = Symptomatic.
• Module enabled if distance visual acuity (left or right eye) ≥ 6/18.
• Module enabled if near visual acuity is reduced.
• Module hidden / disabled if none of the above conditions are met.
