NCD screening form once filled must be available after 1 year , it is allowing to fill any number of times now.

Acceptance Criteria:

1. We are registering the beneficiary and filling the CBAC form in NCD module for the 1st time.
2. Once CBAC form is filled in NCD screening, then that details must be available after 1 year for that particular beneficiary, so that details need not to be filled again.
3. We can maintain the history of beneficiary who is coming for the 1st time and utilize the details for future visits.
