Description:
When an abortion is recorded from the ANC module, the Weeks of Pregnancy shown in the Abortion List does not match the value displayed in the CAC form. Additionally, important fields such as Abortion Date, Abortion Type, and Facility are not getting populated in the CAC form.

Steps to Reproduce:

1. Open the application.
2. Go to the ANC module.
3. Record an abortion entry for a beneficiary.
4. Navigate to the Abortion List and check Weeks of Pregnancy.
5. Open the CAC form for the same beneficiary.
6. Verify Weeks of Pregnancy, Abortion Date, Abortion Type, and Facility fields.
