CG[2.8]-NCD Follow-up form submitting without mandatory fields and multiple submissions increasing visit count

      Issue Description

      In the NCD Follow-up form, the form is getting submitted even when mandatory fields are not filled. Also, the form can be submitted with null value multiple times, and each submission incorrectly increases the follow-up visit count.
