While entering details in a text box and scrolling down the page, selecting a radio button causes the application to automatically redirect back to the text box field. This issue is observed across multiple modules and disrupts the user flow during form entry.

Modules Affected:

• Maternal Health → ANC Visits → Add ANC Visit
• Maternal Health → PNC Visits → Add PNC Visit
• Child Care → HBNC
• Child Care → HBYC

Steps to Reproduce:

1. Navigate to any of the above modules
2. Open the Add Visit/Form screen
3. Click on a text box and enter some details
4. Scroll down the page
5. Click on any radio button
6. Observe the page behavior
