The application is crashing when attempting to submit the Check SAM form under the "Children under 5 years" module. 

Steps to Reproduce:

1. Open the application
2. Navigate to "Children under 5 years" module
3. Open the Check SAM form
4. Fill in the required details
5. Click on Submit

Expected Result:
The form should be submitted successfully, and the user should be redirected to the next screen or confirmation page.

Actual Result:
The application crashes immediately upon clicking the Submit button.
