Changes related to Maternal Death in PNC:
Following 2 changes are required:

1. In PNC Visit , Rename "Mother Death" to "Is the Mother alive?"

and default value is "Yes"
If "No" is selected collect death details.

2. In "Probable cause of Death":
Rename "Probable cause of Death" to "Reason for Death"

in "Probable cause of Death" dropdown remove item "Any Other" and add "Other Maternal Death" (Rename "other" to "Other Maternal Death".

No need of "Other Death Cause" field. (Remove it)
