Missing Validation Error & Calendar Not Opening for Follow-up Date in TB Confirm Form

      Description:

      In the TB Confirm Form, multiple issues are observed:

      1. Validation errors are not displayed even when required fields are left empty or incorrect data is entered.
      1. The calendar is not opening for selecting the Follow-up Date, preventing users from entering the required date.
