In the NCD module, multiple functionality issues are observed:

1. In the NCD Referred List, the follow-up form is not visible, preventing users from performing follow-up actions.

1. The following buttons are not clickable / not responding:

• • Register Wife

• • Register Husband

• • Eye Surgery

These issues are blocking user actions and impacting workflow in the NCD module.
