Steps to Reproduce (TB Form Crash):

1. Login to the application

1. Navigate to TB Screening Form

1. Fill in all required details

1. Click on Submit

1. Observe the app crash

Expected Result:

• TB Screening Form should submit successfully without crashing.

• Delivery Outcome data should sync properly and be visible after sync.

Actual Result:

• App crashes on submitting TB Screening Form.

• Delivery Outcome records fail to sync correctly.
